Incisional hernia repair by fascial component separation: results in 128 cases and evolution of technique.

چکیده
BACKGROUND Most ventral incisional hernias are repaired using 1 of 2 principal techniques: (1) prosthetic repair (open or laparoscopic) and (2) primary reconstruction by fascial component separation. Primary midline restoration provides physiological advantages, and avoidance of mesh may reduce complications. This report describes 128 cases of incisional hernia repair by fascial release. Evolution of the technique produced modifications and fewer complications. METHODS Fascial component separation was performed either by "classic" technique (broad skin flaps) in group 1 and by "perforator preservation" (fascial release through separate inferolateral incisions) in group 2. RESULTS Mortality was .75% (1/128). Major complications occurred in 7 patients (5.5%). Total recurrence rate is 16% (21/128) with major recurrences in 9.3% (12/128). Both groups were statistically equivalent in demographics, comorbidities, and recurrences. Group 1 had significantly higher rates of skin necrosis (P < .001) and chronic pain (P = .003). CONCLUSIONS Fascial component separation can provide satisfactory results in uncomplicated incisional hernias, but skin necrosis is prohibitive without perforator preservation.

Botulinum Toxin A as an Adjunct to Abdominal Wall Reconstruction for Incisional Hernia
BACKGROUND Repair of large incisional hernias remains a surgical and costly challenge. Temporary paralysis of the lateral abdominal wall muscles with topical administration of botulinum toxin A (BTA) is a new therapeutic concept, which may obviate the need for component separation technique (CST) for repair of large incisional hernias. Current literature on the administration of BTA as adjunct ...
متن کاملRives technique is the gold stardard for incisional hernioplasty. An institutional experience.
AIM We report our clinical experience with incisional hernia surgery and we retrospectively analyze the outcomes obtained with the different techniques of repair used, confirming that Rives-Stoppa procedures actually represent the gold standard for incisional hernia. MATERIAL OF STUDY 334 patients were observed for incisional hernioplasty at our Department of Surgery from 1996 to 2007. They w...
متن کاملA study of intraabdominal pressure modification in "component separation" technique for repair of incisional hernia.
INTRODUCTION Incisional hernias can be treated with laparoscopic and laparotomic surgery. Laparoscopic surgery can be made without the use of mesh when performing component separation technique. This technique allows to limit the adverse effects to foreign body and promotes a tension-free closure. We studied intravescical pressure changes during intervention in order to quantify intrabdominal p...
متن کاملLaparoscopic versus open incisional hernia repair: An institutional experience
Laparotomy performed for surgical access usually heals quickly and without complications, leaving a stable scar. An exception to this is the rare (< 1%) occurrence of acute separation of the sutured abdominal walls during the postoperative phase – known as acute wound dehiscence or burst abdomen – and the more frequent (> 20%) occurrence of chronic wound dehiscence with the formation of a herni...
متن کاملHybrid technique for postoperative ventral hernias – own experience
INTRODUCTION There are many techniques which may be involved in abdominal hernia repair, from classical to tension-free. Treatment of complicated hernias has undergone evolution. Many surgeons consider the laparoscopic method as a method of choice for incisional hernia repair. Sometimes miniinvasive repair of complicated hernia is not so easy to perform. We are convinced that selected patients ...
